 Private Duty Care

 

Why this option?

 

Private Duty is an excellent option for individuals who require more flexibility. After exploring every other funding option, individuals may pay out-of-pocket for some or all of their services. Home4Life In-Home Services reserves the right to meet any income need for services.

 

Home4Life In-Home Services conducts "No-Obligation" In-Home Assessments which are scheduled within 24 hours of inquiry. During the initial assessment process, the client will discuss their current level of independence and will get assistance in exploring areas of needed care. The client and the coordinator will have an opportunity to agree on an individualized care plan; this plan will be followed by the Professional. The care plan details specific services/scheduling that meets the need of the client. This plan may be revised at any time by the client/appointed persons.

 

Professionals are responsible for documenting the care provided each day. Clients/appointed persons will be responsible for signing documentation each day. If for any reason, the client is not satisfied with the Professional's care, the client may request another Professional. If for any reason, the Professional is not available to provide care, a backup Professional will be provided. The client will always be informed of who will be in their home.

 

Levels of care provided include:

 

Personal Care, Homemaker Services, Chore Services, In- Home Respite and Nurse Supervision

 

Professionals will not provide services for other persons residing in or visiting the home unless and assessment has been completed on that individual and a care plan has been established.

 

Contract for Service

 

A contract for service is required prior to the delivery of service. There is no min/max amount of time required for contract of service; we provide short-term and long-term care. Contract for service must be signed by the client and the agency.

 

Cost for Care

 

Please request a free initial In-Home Assessment to be quoted an accurate cost for care. Home4Life In-Home Services provides services 24/7 and does not charge additional fees holidays or mileage.

 

Billing

 

Services contracted to be delivered between the 1st and 15th of the month must be paid no later than the 5th; Services contracted to be delivered between the 16th and last day of the month must be paid no later than the 20th. Payments accepted: Visa, Mastercard, American Express, Discover, Long Term Care Insurance/Medical Insurance, Cash and Money Orders. Personal Checks are not accepted. Home4Life In-Home Services maintains receipt history for each client and is available upon request.

 

Cancellation Policy

 

Home4Life In-Home Services may have to suspend or discontinue services if the client's care becomes unavailable at the agency or if the client may need to be transferred to a hospital or a residential facility.

 

Upon service delivery, the client understands and agrees that he/she is legally obligated to pay any balance due to the agency for services provided up to and including as applicable the date on which the client leaves through the end current billing month.

 

Home4Life In-Home Services reserves the right to involuntarily discharge a client from the agency based on several factors. The client will receive a handbook which outlines details of how the agency is governed.
