Home4Life In-Home Services is a current provider for the "HCY" Program.
What is the "HCY" Program?
The "HCY" program provides service coordination and authorization for medically necessary services for MO HealthNet recipients with special health care needs for children under 21 years old.
Our Services include:
This program is available to all MO HealthNet (formerly Medicaid) eligible children residing in Missouri who are in need of medically necessary services from MO HealthNet providers.
The participant must:

Be a Missouri resident

Under 21 years old

Need medically necessary services

Be enrolled in MO HealthNet
